An individual that is assigned to your family will be your point of contact for Reviv. This person will oversee communicating with you about planning activities for your family in the Reviv room or in your home, dropping items off to you when needed, local happening, etc.
Every family will have a different service plan, so every support working will be able to adjust the support that is given, based on your family’s unique needs at the moment. After your meeting, you should expect an email with contact information for your worker and should expect them to contact you within one week of being assigned. If you should not hear from them, you can contact the Family Support Coordinator for further assistance.
